Deep gluteal syndrome describes sciatic nerve irritation within the deep space behind the hip. Piriformis syndrome is one possible subtype, not the explanation for every buttock pain presentation.

Common symptoms

1. Deep buttock pain aggravated by sitting
2. Pain or tingling down the posterior thigh
3. Symptoms with running or stairs
4. Pain in selected hip positions

Why accurate diagnosis matters

Lumbar disc, spinal stenosis, sacroiliac and hip tendon disorders may mimic it, so spinal and hip sources must be screened.

PhysioTech assessment

We assess symptom behaviour, movement, strength, sensation and aggravating factors, while screening the spine, nearby joints and peripheral nerves. No single test or scan determines the plan.

Physiotherapy management

1. Modify aggravating sitting and loading
2. Restore relevant hip and trunk movement
3. Use tolerable neural mobility when indicated
4. Progress hip strength and movement control
5. Grade the return to running and sport
Adjuncts such as manual therapy, dry needling, kinesio taping, electrotherapy or therapeutic ultrasound are used only for a defined supporting goal. They do not replace assessment, exercise, graded loading or outcome review.

When medical review is needed

Foot weakness, bowel or bladder change, saddle numbness or rapidly worsening pain requires urgent medical assessment.

Book an assessment at PhysioTech

If symptoms affect sleep, work, walking or sport, book an individual assessment at PhysioTech in Zahraa El Maadi to identify the likely driver and build a measured plan.

FAQ

Is it the same as sciatica?

It can mimic sciatica, but sciatica is a symptom pattern with multiple causes.

Is dry needling the main treatment?

No. It may be an adjunct; assessment and graded rehabilitation remain central.
Note: Educational content only; it does not replace diagnosis.
