Pain when lifting your arm, reaching overhead, dressing or lying on one side does not automatically mean that a tendon is torn. Many cases fit the clinical label subacromial shoulder pain, involving symptoms around the rotator cuff and nearby tissues. “Impingement” is a common term, but diagnosis should not rely on one scan or one special test. It requires a history, movement and strength assessment, and consideration of how symptoms affect daily activity.

Common symptoms

  • Pain over the front or outer shoulder.
  • Pain with overhead reaching or lifting away from the body.
  • Night pain, especially when lying on the affected side.
  • Reduced confidence or strength during lifting.
  • Difficulty dressing or reaching a high shelf.
Similar symptoms can arise from the neck, frozen shoulder, rotator cuff tears or joint problems, so an individualized assessment matters.

Assessment at PhysioTech

At PhysioTech in Zahraa El Maadi, assessment reviews symptom behavior and recent load changes, measures range of motion, tests rotator-cuff and scapular muscle capacity, and screens the neck when pain, tingling or numbness travels into the arm. Imaging may be considered after major trauma, sudden marked weakness or an unexpected clinical course, but it is not required for every first visit.

Physiotherapy management

Load modification

Temporarily reduce the most provocative tasks while keeping safe movement and activity. Prolonged complete rest can increase stiffness and deconditioning.

Restore movement

Mobility exercises are selected for the patient’s specific restrictions. Manual therapy may support short-term comfort and movement when appropriate, but it does not replace exercise.

Progressive strengthening

Rehabilitation develops rotator-cuff and scapular strength, then progresses toward pushing, pulling, lifting and overhead work according to the patient’s goals. Progression is more important than one “perfect” exercise.

Return to work or sport

Readiness is judged by load tolerance, strength, control and performance of the required task without a sustained symptom flare.
Kinesio taping, electrotherapy or therapeutic ultrasound may be considered as adjuncts for selected patients. They do not replace assessment, exercise therapy or graded loading.

Red flags

Seek urgent medical advice after a fall with deformity, sudden inability to lift the arm, severe increasing pain with fever or redness, chest pain or breathing difficulty, or progressive weakness and numbness.

FAQ

Does shoulder impingement mean bone is cutting the tendon?

Not necessarily. Symptoms are influenced by tendon and muscle capacity, movement, sleep and load. Assessment identifies the most relevant factors.

Must I stop the gym?

Usually not completely. Angles, weights and training volume can be modified and then progressed.

How long does recovery take?

It varies with symptom duration, strength, sleep, workload and adherence. We track movement, strength and function rather than promise a fixed timeline.
