Forced thumb abduction during a fall or ball impact can injure the ulnar collateral ligament at the thumb MCP joint. Often called skier’s thumb, it ranges from a mild sprain to complete rupture that compromises pinch and grip.
The following information is educational and does not replace an individual examination, especially after major trauma or when warning signs are present.

What Is the Condition?

The UCL stabilises the inner side of the thumb MCP joint during grasp. In a complete tear, the ligament can displace superficial to the adductor aponeurosis as a Stener lesion, limiting spontaneous healing.

Symptoms You May Notice

• Inner thumb-base pain and swelling.
• Weak pinch between thumb and index finger.
• Instability opening a jar or holding a key.
• Bruising after trauma.
• Pain when the thumb moves away from the hand.

Causes and Contributing Factors

A fall with thumb abduction, ball impact or forceful twist are common. Repeated chronic stress can gradually stretch the ligament.

Conditions That Can Look Similar

Thumb-base fracture, volar-plate injury, thumb CMC osteoarthritis, de Quervain tenosynovitis and extensor-tendon injury.

How Is It Assessed at PhysioTech?

Pain site, circulation and sensation are checked, and a physician carefully assesses stability against the other side. X-ray can exclude fracture, while ultrasound or MRI may help with a suspected complete tear.

What Do We Measure?

Pain and stability, thumb range, pinch and grip strength, writing, grasp and sport tasks after clearance.
We record a meaningful task such as walking, running, stairs or reaching, the response during it and what happens later and the next day. Movement, strength, capacity and task quality are compared over time rather than judging progress only by short-term relief in the session.

When May Imaging or Medical Referral Be Needed?

Clear instability, a mass or severe pain after injury, suspected fracture, numbness or colour change needs prompt review; complete tears may need early surgery.
Imaging is not required for every patient. It becomes important after major trauma, with warning signs, when progress is unexpectedly poor, or when the result would change loading, referral or surgical decisions. Imaging decisions are coordinated with the appropriate physician.

Thumb UCL Injury Treatment at PhysioTech

1. Protect the Area and Manage Load

Protect the joint in an appropriate splint and avoid resisted thumb opening until injury grade is established.

2. Restore Movement and Control

After clearance, restore thumb motion without excessive side stress and manage swelling and scar.

3. Build Strength and Capacity

Progress pinch, grip and thumb-muscle strength before adding precise functional tasks.

4. Return to Activity or Sport

Return to work or sport with appropriate protection and reliable grasp without instability or increasing pain.
A central part of care is understanding why each exercise was chosen and how to adjust the dose. We agree on an acceptable symptom response, then progress repetitions, resistance, speed or task complexity. We do not promise a fixed recovery date because injury severity, general health and activity demands vary.

The PhysioTech Treatment Philosophy in Cairo

At PhysioTech Clinic in Zahraa El Maadi, Cairo, care starts with assessment rather than an imaging label alone. We connect symptom behaviour with movement, strength, capacity and work or sport demands, record clear baseline measures and retest them. Manual therapy, dry needling, kinesio taping, electrotherapy or therapeutic ultrasound may be used for a defined purpose, but they do not replace education, therapeutic exercise and graded loading.
The injured ligament is not repeatedly stress-tested. Early diagnosis protects healing opportunity, then rehabilitation restores stability and function.

Red Flags That Need Prompt Medical Review

Thumb deformity, numbness, coldness or colour change, an open wound, major grip loss or rapidly increasing swelling.

Frequently Asked Questions

Does every injury need surgery?

No. Lower-grade injuries may heal in a splint, while complete tears or a Stener lesion may require repair.

When can movement start?

It depends on grade and treatment; unprotected early motion can compromise a complete tear.

Does pain alone show the grade?

No. Stability, examination and selective imaging matter more.

When Should You Book an Assessment?

If the thumb feels unstable or grip weakens after injury, book a hand assessment at PhysioTech in Zahraa El Maadi, Cairo.
